Application Forms

All essential forms for admission are available in Adobe Acrobat Portable Document Format (PDF). In order to viewthe forms, you must have the Adobe Acrobat Reader software, Download Adobe Acrobat Reader (free). Instructions are included in the PDF documents. In order to print the forms, you will need a laser or inkjet printer.

Freshman Admission

You are considered a freshman if you are any of the following:

  • a first-time college student;
  • have attempted fewer than 12 college-level credits;
  • a traditional-age first time high school graduates with college credit;
  • a traditional-age freshmen with GED;
  • a non-traditional student with a High School diploma or GED and no college credits.

If you met any of the above, please complete a Freshman application.

Transfer Admission (College of Technology)

If you have attended another college and attempted 12 or more college-level credits you are considered a transfer student in the admission process; please complete and submit a transfer application.

You are considered an international student if:

  • you are not a U.S. Citizen, permanent resident, member of a Jay Treaty Tribe or refugee; or
  • you are living in the U.S. under an F, B, H, J, L or M visa.
